香港政府【慢性疾病共同治理先導計劃】​

香港特區政府於2023年11月宣布推出「慢性疾病共同治理先導計劃」, 來應對人口老化和慢性疾病病患率上升為醫療系統帶來的新挑戰,將醫療健康架構的焦點從治療轉向至預防疾病。 未來政府  致力加強以地區為本的基層醫療健康服務,以期扭轉現時「重治療、輕預防」的醫療體制和觀念。

Q: What is Chronic Disease Management?

According to the World Health Organization (WHO), chronic diseases are defined as “diseases of long duration and generally slow progression” that “are the result of a combination of genetic, physiological, environmental, and behavioural factors.” Chronic diseases usually have the following characteristics:

  • They persist over time and cannot be completely cured
  • Their progression is slow, with periods of exacerbation and remission
  • They significantly impact the quality of life and functioning
  • They require long-term management and treatment

Chronic diseases are a leading cause of death and disability worldwide, accounting for approximately 70% of all deaths globally. In Hong Kong, the percentage of people with chronic health conditions was 31% (around 2.2 million) in 2020/21. About 55% of deaths in Hong Kong are attributable to chronic diseases. Key to managing chronic diseases is early diagnosis, effective treatment plans, and ongoing monitoring.
